Community Health and Wellness Division

The Community Health and Wellness (CHW) Division is dedicated to improving the physical and mental health of Fort Bend County residents of all ages.  CHW supports Fort Bend County residents in preventing, detecting and managing chronic conditions — including heart disease, high blood pressure, diabetes and cancer. CHW also plays a critical role in identifying and addressing mental health challenges to improve the overall mental well-being of our community.

In addition to providing education and community outreach, CHW collaborates closely with the Epidemiology Division to track trends in chronic disease and mental health trends as well as other related needs across the county.
