What We Do
The Chronic Disease Program provides a range of services to members of the Fort Bend County community to help prevent chronic diseases, support the management of diagnosed chronic illnesses and promote overall health through education. The program focuses on managing these illnesses through self-care, symptom management and enhancing overall health and well-being.

What services are provided to the community?
- Hemoglobin A1c screenings
- Cholesterol screenings
- Blood pressure checks
- Diabetes, pre-diabetes and high blood pressure health education
- Nutrition education and counseling
- Linkage to care for patients without an established primary healthcare provider

- Adults with a diagnosis of pre-diabetes, diabetes or high blood pressure
- Youth and their families
- Pregnant women and infants
- Older adults
Where are these services provided?
- Services are provided at the Missouri City and Rosenberg Annex locations via referral from other Fort Bend County Health and Human Service (FBCHHS) programs (i.e., Tuberculosis (TB), Human Immunodeficiency Virus (HIV) and Sexually Transmitted Infections (STI) Prevention programs as well as Mental Health and Social Services, etc.
- Health and Wellness Fairs in the Fort Bend County community.
- Other Community Events, including but not limited to presentations at community-based organizations, faith-based organizations, schools—including preschool, elementary, middle, high school and higher education—across all of Fort Bend County.
